Change one thing at a time

The only reliable way to identify a trigger is to change a single product and wait several days. Changing detergent, soap and cream all at once tells you nothing.

Keep a simple note of what changed and when. A week later you will not remember.

The usual suspects

Fragrance is the most common culprit, followed by harsh detergents, fabric softener, wipes with alcohol, and new clothing that has not been washed before first wear.

Wool and synthetic fabrics against the skin irritate some babies. Cotton next to the skin is the safer default.

Patch-test anything new

Apply a small amount to a small patch of skin and wait 24 hours before using it more widely. This costs a day and can save a fortnight of unhappy skin.

Do this even for products marketed for sensitive skin — the label is a claim, not a guarantee for your particular baby.

Where the rash is tells you something

A rash confined to the nappy area points to the nappy, wipes or cream. One that follows clothing lines points to detergent or fabric. One on the face and hands may be food, drool or something touched.

Photograph it. Rashes change, and a picture helps your doctor considerably.

When to see a doctor

See your paediatrician if the skin is broken, weeping, blistered or looks infected; if the rash spreads quickly; if it comes with a fever; or if your baby seems unwell or is scratching enough to disturb sleep and feeding.

Any swelling of the face, lips or tongue, or any difficulty breathing, is an emergency — seek immediate medical help. Do not wait to see whether it settles.
